PART I: OUT OF CHARACTER INFORMATION
NAME: We’re not talking legal name on the birth certificate, sweetpea – just what you liked to be called on the Internet is fine!

AIM/E-MAIL: Although AIM is hardly essential, having a screenname enhances your membership by being able to join game chats to build community and talk with your fellow players! However, having an e-mail is an absolute must. A Gmail account for accessing Gdocs? A plus.
CDJ: Again, not required to post if you wish to maintain privacy or lack a CDJ on your person, but knowing these journal accounts are handy in the case that someone may wish to friend your lovely face!

PART II: CHARACTER INFORMATION
NAME:
AGE, BIRTHDAY: Please remember: nobody below the age of thirteen, as would be reflected in the Children’s Online Privacy Protection Act of 1998. Otherwise, everyone is fair game.
SEXUALITY:
OCCUPATION: Student, policeman, small business owner, professor? None of these things? Talk about it here.

ORIGIN OF BIRTH/NATIONALITY: Foreign nationals must list their country of citizenship; otherwise, please list city and state of birth.
NATIONAL IDENTITY CARD TYPE: Choices include “Northeast, Southeast, Great Lakes, Midwest, Southwest, West, Commonwealth: COMMONWEALTH,” and “Foreign National: COUNTRY.”
RESIDENCE: The above does, in fact, affect the below; please indicate what neighborhood your character presently lives and the type of accommodations they presently have – if any at all.
POLITICAL AFFILIATION: Rate on a general litmus between liberal and conservative, with special attention to rebel tendencies (if applicable).

IMPORTANT NPCs/FAMILY MEMBERS: List any immediate family members or important people in your character’s life that make an impact on his/her day-to-day activities. Name, age, and relation to the character are the only things required; anything else is merely for the applicant’s benefit!
HISTORY: Please go into detail here; the history of a person is the foundation of self, as it were. Full paragraphs, please.

PERSONALITY: Same as above; full paragraphs, go into as much detail as you find necessary to flesh out the character, etc. Dissertations not expected, but one-liners are frowned upon.

PB: Your actor of choice for the faux-role and a link to a relatively high-quality image of this individual! Multiples are a boon, but not demanded.

PART II-B: CHROMOSOME 17-B ADDITIONS
PLEASE NOTE, 17-B APPLICANTS: This section is not to be released on your public profile at all! This section must be e-mailed to the moderator until it is made reasonably public that Chromosome 17-B characters exist! Questions? Ask that moderator of yours!

POWER: What supernatural power(s) has/have your character developed and how has your character’s former physical flaws attributed to this new adaptation? For those at a loss, appropriate powers would include infrared vision for a former blind person, echolocation for the formerly deaf, telekinesis for a former stroke patient, and so on. Please feel free to make infinitely more creative power manifestations than this; these are merely purposefully bland examples! If you have any questions, please contact the moderator.
STRENGTH LEVEL: In short: how much punch does your character now pack? If you need a reference, please rate on a level of 1 (meek, mild, barely manifested) to 10 (Hulk smash with brain!). If you have plans for your character’s powers to be intense later on in the game but mild to undetected at the start, please note as such here.
LEVEL OF CONTROL: Was your character overwhelmed by the sudden manifestation of these powers or have they slowly developed over time? How well does s/he handle controlling these powers in public? If there is a lack of control, does s/he make a point to hide these powers? Are there any adverse side effects to attempting to stifle these phenomena in public?
POWERS HISTORY: Think of this as a way you can cover the parts of your character’s history that would normally contain blurbs about how they found out that they were positive for Chromosome 17-B if the Mod wasn’t being a secretive hardass about it. Is your character even aware of his/her power potential at the moment? If so, how did s/he find out? Was someone present? If so, did s/he make sure that the witness(es) were incapable of remembering afterward? If not, then how many people other than the character know?

PART III: MISCELLANEOUS QUERIES
The following section is separated into a series of required and non-required (but fun!) questions:

REQUIRED:

DO YOU HAVE INTENT OF INVOLVING YOUR CHARACTER IN ANY REBELLION ACTIVITIES? Self-explanatory, but further elaboration as to the how and why of such a decision is always a plus if you feel so inclined.
HOW DO YOU FEEL ABOUT CHARACTER MORTALITY? We’re a messy, violent game; feasibly, deaths are possible and not just through the usual inactive character rush-and-go murder. If you really and honestly feel as if you’re not comfortable with the possibility of your character’s death, please say so here! On the other hand, if you’re interested in killing this character off for plot reasons, also list here!
WHAT ARE YOUR PLANS FOR PLOT? Guess who has two thumbs and is interested in integrating your plans for your character with overall game plot? Hint: it’s this guy. If you have plans for your character right now, share with the class! It’s okay if you don’t, though.

WHAT IS YOUR CHARACTER’S THEME SONG? Music is really important to this game in one way or another; pinning a song to the spirit of your character is encouraged, as a result. If you have a wild hair, feel free to list a soundtrack!
HOW DOES YOUR CHARACTER FEEL ABOUT NEW YORK? Native or refugee, the Big Apple is your character’s present home. Do they love New York or find it to be a symbol of everything that’s gone wrong in the past two years? Do they want to fix it or deal with it until they can get the hell out of dodge?
WHAT IS YOUR CHARACTER’S BIGGEST PRIORITY? Family, friends, terraforming?
THERE’S A RIOT ON THE NUMBER 2 LINE IN THE BRONX – HOW DOES YOUR CHARACTER REACT? Horrified? Disgusted? Jealous?
